Reference the form you received in the mail and find the URL at the bottom of that form.
Follow the steps below and you will be able to get to your sponsor's ordering page.

Open a new internet browser window. At the top of the page you will see the URL/web address field.
Copy the exact URL from the form into the web browser URL/web address field.
Don't forget that you must add the .html to the end of the web address and that some addresses contain underscores ( _ ), found by pressing shift and the hyphen button (which is located between number "0" and the add/equals "+ =" button).
Press the "Enter" or "Return" key on your keyboard, or click the "Go" button on your browser (generally located next to the URL web address field.
